• If you are a health plan member, including Medicare Advantage plans, specimen collection services would likely be covered by your health plan if you visit a patient service center, and you may not be eligible to seek reimbursement from your health plan if you elect to use Collection Services provided by Getlabs.
• If you are a homebound patient, specimen collection services may be available from LabCorp (and covered by insurance) and by continuing you elect to receive and self-pay for Collection Services provided by Getlabs.
• If you are a Medicare beneficiary, you understand that specimen collection services would be covered at LabCorp patient service center and by continuing you elect to receive and self-pay for Collection Services provided by Getlabs.
• All patients have the option to receive services from LabCorp via a visit to LabCorp patient service center and by continuing you elect to receive and self-pay for Collection Services provided by Getlabs.
