Test Includes

Serum protein electrophoresis with relative percentages and quantitation of albumin; α1-, α2-, β-, and γ-globulin fractions; graph of fractions; quantitation of total protein and M-spike if present; quantitation of serum free κ and λ light chains plus κ:λ ratio

Expected Turnaround Time

3 - 5 days

Specimen Requirements

Specimen

Serum

Volume

2.5 mL

Minimum Volume

1.2 mL

Container

Red-top tube or gel-barrier tube

Collection

Sample should be allowed to clot and the serum separated as soon as possible to prevent hemolysis.

Storage Instructions

PE: Stable when covered at room temperature 25°C for four days; stable refrigerated at 2°C to 8°C for two weeks; stable frozen at -20°C for six months.

FLC: Refrigerate; samples are stable for 21 days at 2°C to 8°C. For longer storage, freeze at -20°C.

Causes for Rejection

Lipemia; hemolysis; plasma specimen

Test Details

Use

Detect and identify monoclonal immunoglobulin gammopathies

Methodology

Protein electrophoresis: agarose media; free κ and λ light chains: turbidimetric quantitation

Additional Information

Elevated serum levels of monoclonal FLC are associated with malignant plasma cell proliferation (eg, multiple myeloma), primary amyloidosis, and light chain deposition disease. Raised serum levels of polyclonal FLC may be associated with autoimmune diseases such as systemic lupus erythematosus.
