Monoclonal protein testing plays a central role in the diagnosis and monitoring of plasma cell disorders such as multiple myeloma. This presentation reviews the clinical application of serum free light chain (sFLC), serum protein electrophoresis (SPEP), immunofixation electrophoresis (IFE) analysis, and highlights the technical challenges associated with electrophoretic and immunoassay-based methods, including subjective interpretation, therapeutic interference, antigen excess, and monoclonal vs. polyclonal sFLC detection. The session also explores how advanced mass spectrometry approaches, such as MALDI-ToF MS, may overcome some of the challenges with electrophoretic methods.
