Interstitial lung disease-associated antibody testing and positivity rates in systemic autoimmune rheumatic diseases

04 May 2026

  • Interstitial lung disease (ILD) is common in systemic autoimmune rheumatic diseases (SARDs) and is associated with increased morbidity and mortality.
  • ILD prevalence is estimated at ~52% in systemic sclerosis, 41% in myositis, 23% in Sjögren’s disease, 21.4% in rheumatoid arthritis, 14.5% in systemic lupus erythematosus, and 56% in overlap syndromes.
  • While early detection of ILD is critical, diagnostic imaging is often limited to high-risk patients. Antibody testing is a cost-effective approach for identifying patients at increased ILD risk.
  • We evaluated testing and positivity rates of ILD-associated antibodies across SARDs to assess current screening practices.
